Organisation Registration Form: Explanation
Organisation name: The official name of your organisation as you would use it in official correspondence.
Region: Select the region where your organisation is located.
Organisation type: Select the type of organisation from the list or Other if appropriate
Contact person: The name of the person who can normally be contacted about pet adoption matters.
Phone: Phone number of your organisation or, alternatively, the number of the pet adoption contactperson.
Mobile phone Mobile phone number of your organisation or, alternatively, the number of the pet adoption contactperson.
Fax: Fax number of your organisation.
Email: Email address of your organisation. Note that this MUST be an address that is read on a REGULAR basis, since it will be used to refer prospective owners to.
Address: The official address of your organisation as you would use it in official correspondence.
Town: The town where you are located.
Web site: Web site of your organisation which MUST include the http:// portion. You may copy this from the URL bar in your own browser.
Bank name: Name of your bank.
Bank branch: The branch name of your bank.
Bank account: Your bank account number. Note that this information will be used e.g. when an adoption fee applies.
